چکیده

Introduction Proportional Assist Ventillation (PAV) is a patient and user friendly mode of spontaneous mechanical ventilation which uses dynamic feedback of respiratory parameters to support the effort and assess the consequent workload overcomed by the patient ́s respiratory muscles to which diaphragm contributes the most. The relationship of Pmus [1] (respiratory muscle pressure) and diaphragmatic function (displacement-excursion/ thickening ratio) has not however been adequately investigated.

Limited predictability of maximal muscular pressure using the difference between peak airway pressure and positive end-expiratory pressure during proportional assist ventilation (PAV)

BACKGROUND If the proportional assist ventilation (PAV) level is known, muscular effort can be estimated from the difference between peak airway pressure and positive end-expiratory pressure (PEEP) (ΔP) during PAV. We conjectured that deducing muscle pressure from ΔP may be an interesting method to set PAV, and tested this hypothesis using the oesophageal pressure time product calculation. ME...

Control of breathing during sleep assessed by proportional assist ventilation.

We used proportional assist ventilation (PAV) to evaluate the sources of respiratory drive during sleep. PAV increases the slope of the relation between tidal volume (VT) and respiratory muscle pressure output (Pmus). We reasoned that if respiratory drive is dominated by chemical factors, progressive increase of PAV gain should result in only a small increase in VT because Pmus would be downreg...

Non-invasive proportional assist and pressure support ventilation in patients with cystic fibrosis and chronic respiratory failure.

BACKGROUND Patients with advanced cystic fibrosis can benefit from non-invasive positive pressure ventilation (NPPV) for the treatment of acute decompensation as well as for the management of chronic respiratory failure. This study was undertaken to compare the physiological effects of non-invasive proportional assist ventilation (PAV) and pressure support ventilation (PSV) on ventilatory patte...

Effects of respiratory muscle unloading on exercise-induced diaphragm fatigue.

We previously compared the effects of increased respiratory muscle work during whole body exercise and at rest on diaphragmatic fatigue and showed that the amount of diaphragmatic force output required to cause fatigue was reduced significantly during exercise (Babcock et al., J Appl Physiol 78: 1710, 1995). In this study, we use positive-pressure proportional assist ventilation (PAV) to unload...

Can proportional ventilation modes facilitate exercise in critically ill patients? A physiological cross-over study

BACKGROUND Early exercise of critically ill patients may have beneficial effects on muscle strength, mass and systemic inflammation. During pressure support ventilation (PSV), a mismatch between demand and assist could increase work of breathing and limit exercise. A better exercise tolerance is possible with a proportional mode of ventilation (Proportional Assist Ventilation, PAV+ and Neurally...
